General Practitioner or Geriatrician for Innovative Elderly Care Model

About the role

We are seeking qualified General Practitioners or Geriatricians to join a VC‑backed initiative that is redesigning care for elderly patients with multiple chronic conditions. The position can be full‑time or part‑time and may be performed remotely, allowing integration with existing private practice or hospital activities.

Key responsibilities

• Provide geriatric tele‑expertise and clinical support to frontline professionals such as GPs, private nurses, nursing homes, and medico‑social structures.
• Conduct targeted teleconsultations and intervene in unplanned or high‑risk situations to prevent care disruptions and avoidable hospitalisations.
• Coordinate care pathways and contribute to innovative territorial systems, including “nursing home without walls” concepts.
• Participate in regular multidisciplinary meetings, peer discussions, and continuous improvement of the care model.

Required profile

• Registered General Practitioner or Geriatrician with the French Medical Council.
• Strong interest in geriatrics and managing complex, multi‑morbid patient cases.
• Willingness to work within a coordinated, anticipatory care framework.
